Jack Arthur Smith, stockman, age 19, born at Moore Park, Sydney, NSW; Enl.10Nov1914 at Tambo, Qld, in 5LHR, with previous service of 1 year in School Junior Cadets; Emb.09Feb1915; TOS 2LHR at Alexandria 02Jul1915; various hospitalisations for enteritis and a hernia; RTA 23Sep1916; Disch.24Nov1916 Medically Unfit; Issued a statutory declaration on 14Apr1920 stating that his correct name was Peter Joseph Noonan and he had enlisted under the false name of Jack Arthur Smith.
