Together with OHMS medal box, and small individual medal packets, addressed to Mr D B Shaverin, 39 Paramount Court, University St, London WC1.

MID: Supplement to LG 2/10/1942, p4328 - to Engineman David Benjamin Shaverin, LT/JX.109920 - For gallantry, daring and skill in the combined attack on Dieppe (HMS Tormentor) Operation Jubilee.

MID (2nd award): Supplement to LG 11/12/1945, p5996 - to Temporary Lieutenant (E) David Benjamin Shaverin, R.N.V.R. - For distinguished service in NW Europe (HMS Hamilcar) Operation Wind Up (Europe 45).
